Every application for a wholesale delivery license shall be by affidavit of the applicant and shall clearly set forth:
A.

The name of the applicant; if the applicant is a firm or association, the name of each partner or member; and if the applicant is a corporation, designating the officers and directors and address thereof;

B.

The location and address of the principal place of business of the organization;

C.

The type of business to be conducted.
